From a young age, he developed a deep connection to the land; a bond that would last a lifetime. Jack’s home on the farm was lovingly built by his grandfather, laying the foundation for generations of hard work, pride, and resilience. Jack married Elsie over 58 years ago, they were married at his favourite place- Ballarat in Victoria, Jack and Elsie would holiday there often. Together they built a loving family home filled with warmth and laughter. They have two children: Mary and Matthew. Throughout his life, Jack has been known as a kind and loving husband and father. Farming was more than just a job for Jack; it was a way of life. His days were spent tending the land his grandfather once worked on, and he always had a loyal Kelpie work dog by his side, whether rounding up cattle or keeping him company on long days in the paddock. Jack had a special bond with his dogs and couldn’t imagine farm life without one nearby. In his free time, Jack loved trout fishing, enjoying the peace and quiet of nature. He was a big fan of country music, especially Slim Dusty, Jack was also a passionate supporter of the AFL team, the Geelong Cats. Jack always enjoyed a few beers after a hard day’s work and has been a heavy smoker since he was a teenager. He enjoyed hot chocolate every night before bed. Jack took great pride in his flower gardening and the restoration of old furniture. His skill and dedication earned him prizes at local community events. Both Jack and Elsie could always be relied upon to volunteer at many local events and functions. Over the years, Jack faced a few health challenges, including cardiovascular disease, osteoarthritis, and occasional bouts of gout. Jack wears glasses and a hearing aid in his left ear. Every night he would lock the sheds and gates to his farm to ensure the cattle and other animals were safe and secure – a small but significant act that reflected his care and responsibility.Medical summary – diagnosis of dementia: Jack Patient Name: Jack Diagnosis: Alzheimer’s disease, heart disease, osteoarthritis Background: Jack, a long-term resident of Castlemaine in Victoria, was diagnosed with Alzheimer’s disease approximately four years ago. The diagnosis followed concerns raised by his adult children during a visit home after Jack’s wife, Elsie, suffered from a cardiac event and was hospitalised. Jack found Elsie lying on the bathroom floor after her heart attack and was very distressed. Presentation and concerns: Jack’s children observed several significant cognitive and functional issues, including: Poor personal hygiene Word-finding difficulties and short-term memory loss Disorientation within his own home Inability to prepare meals or manage basic household tasks Hesitation, irritability, and anger when support was offered A critical incident where Jack went missing while using the tractor on the farm. He was later found disoriented, cold, hungry, and dehydrated. Further investigation revealed that Elsie had been compensating for Jack’s increasing cognitive decline for some time, which had masked the severity of his symptoms. Impact on Care and Family Decision: Following the diagnosis, a multidisciplinary family conference was held involving Jack and Elsie’s specialists. It was determined that Elsie’s ability to continue caring for Jack was severely compromised due to her own health issues and the complexity of Jack’s care needs. The recommendation was made for Jack to be placed into respite care, with the possibility of transitioning to long-term residential care depending on Elsie’s recovery and ongoing capacity. Current Status: Jack is in respite care. He is having trouble settling into the care facility and misses Elsie. His family remains actively involved in decisions and support.Part 3 – Changed Behaviour – Jack Case Study Participant Instructions Using the Jack Case Study in your Participant Manual, apply the CAUSEd model to explore triggers and strategies to support Jack with his changed behaviour(s).
