reply to this post: Implementing an evidence-based early ambulation protocol in the ICU requires selecting a change theory that supports staff engagement, behavior modification, and sustainable practice. Two theories often used in nursing practice are Lewin’s Change Theory and Rogers’ Diffusion of Innovation Theory. Lewin’s Change Theory consists of three phases—unfreezing, changing, and refreezing. This model focuses on preparing staff for change, implementing the new behavior, and solidifying the intervention within the unit culture. It is straightforward and widely applied in clinical environments. In contrast, Rogers’ Diffusion of Innovation Theory emphasizes how new ideas spread through a social system. It categorizes staff as innovators, early adopters, early majority, late majority, or laggards, recognizing that individuals adopt change at different rates (Butts & Rich, 2022). Rogers’ theory highlights communication patterns, leadership influence, and social dynamics, which are essential when implementing protocols requiring cultural buy-in. For my intervention—a nurse-led early ambulation protocol for extubated ICU patients—Rogers’ Diffusion of Innovation Theory provides a stronger foundation. Successful adoption requires identifying early adopters among ICU nurses, leveraging influential staff to model mobility behaviors, and addressing hesitancy among those concerned about workload or patient safety. This aligns with evidence showing that practice changes involving collaboration and observability spread more effectively in clinical units (Melnyk & Fineout-Overholt, 2023). My preceptor agreed that Rogers’ theory is the better fit. She explained that ICU nurses vary widely in confidence with mobility tasks, and identifying early champions will be critical. She also emphasized that visible positive outcomes—such as reduced delirium and quicker recoveries—will motivate wider adoption. Her insight reinforced that change must happen through peer influence and demonstrated success, not just policy.
