reply to this post: Collecting data from incident reports is done to identify the cause of the event and come up with a “effect” better known as an action plan to try and prevent this from occurring again or an issue similar. The biggest issue I see incident reports complete on the unit I work on is after a fall, it is always done after a fall because the hospital is trying to identify the top reasons patients fall so they can educate staff and come up with an action plan. A new thing added is called hourly rounding which has been showing evidence of reducing falls because the staff is present hourly addressing the main reasons patients get up and fall which are the 5 Ps. That includes the IV pump making noise or beeping, bathroom, personal items, pain and positioning issues which end up in a fall occurring. Now that the hourly rounding has been added on my unit, satisfaction scores are improving simply because patients and family are seeing staff more and feel less neglected because one of the biggest complaints is saying oh I haven’t seen the nurse in a while because as we all know we can get busy. Incident reports are not a part of the patient medical record, it is a separate database for us where we store the information.
